The Consultation Form is a way to provide your medical information so that it can be shared with personal trainers to help them better understand your goals, conditions and provide you with a tailored training program suitable for your health.


When Should You Complete It?
You should complete your consultation form:

  • Before your first session
  • Anytime your health condition changes
  • When booking sessions with a different trainer or for a different training objective


Here's how you can access it: Log in to your account, click on your name, and select Consultation Form. You can complete and keep it filled in beforehand to avoid any delays.


    What Happens If the Questionnaire Is Not Completed?
    If your medical questionnaire is incomplete:

    • Your trainer may need to spend session time collecting basic information. Having complete information helps the trainer understand your needs, plan the session properly, and suggest the right equipment and training approach.
    • In some cases, a session may be delayed for safety reasons



    Below is a guide to help you understand how to fill out the questions in the form:

    • What are your fitness goals?
    Select the goal you are aiming to achieve through the Fitness999 platform.


    • .Do you feel pain in your chest when you perform physical activity?

    Mention if you feel any type of chest pain or heaviness while doing physical activity.


    • In the past month, have you had chest pain when you were not performing any physical activity?

    It might be possible that you don’t feel chest pain while doing physical activity, but in case there was any instance in the past month where you experienced chest pain during rest, mention that as well.




      • Do you lose your balance because of dizziness, or do you ever lose consciousness?
      If you have ever experienced dizziness or loss of consciousness, please mention it here.


      • Do you have a bone, back pain or joint problem that could be made worse by a change in your physical activity?
      If you experience pain in your body or have issues with your joints, please provide details.


      • Is your doctor currently prescribing any medication for your blood pressure or a heart condition?
      If you are taking any medicines prescribed by your doctor for heart problems or blood pressure, mention them here.


      • Do you know of any other reason why you should not engage in physical activity? (i.e. pregnancy)
      If there is any reason you should avoid physical activity, such as pregnancy or heart disease, please mention it here.




      • Kindly tick the checkboxes if you have food allergies to any of the listed food materials. Ignore this section if you do not have any allergies.
      • Select the food preferences that match your taste so that a diet plan can be prepared accordingly.
      • Fill in your personal details, including your name, birthdate, height (in cm), weight (in kg), and a list of medicines you are currently taking.
